Neuro Arts WA Feedback and Complaints Procedure
If you have a concern or complaint about your current NDIS support or services provided by Neuro Arts WA, it’s important to address it. Please fill out the complaint form below, and we will respond within 3 business days.
A complaint may be made anonymously. You can make an anonymous complaint:
(a) By phone: Call us at 0452 387 576 and state that you wish to make an anonymous complaint (so we don’t ask you to identify yourself).
(b) Online form: Fill out the Feedback and Complaints Form and select "I wish to remain anonymous" to ensure your personal details are not requested.
