Service Standards for Neuro Arts WA

1. Purpose

This document outlines the standards for the therapeutic arts services provided by Neuro Arts WA under the guidance of The National Disability Insurance Scheme (NDIS). Neuro Arts WA aims to empower individuals to exercise choice and control in pursuing their goals and planning and delivering their supports.

2. Service

Neuro Arts WA offers arts therapy for individuals and groups.

3. Fees

  • Prices for the services follow NDIS guidelines or are as agreed upon by participants.

  • Payments can be made via bank transfer or cash through NDIS plan managers or directly with the client.

  • Payments must be made within 14 days of the invoice date.

  • Payment requests can be made before or after the relevant services have been provided.

  • Services are exempt from Goods and Services tax.

4. Rights and Responsibilities

4.1 Rights and Responsibilities of the Participant

The Participant agrees to:

  • Inform Neuro Arts WA about their preferences for service delivery to meet their needs.

  • Communicate any concerns about the services being provided.

  • Provide 24 hours notice if they cannot attend a scheduled appointment, failing which the full fee will be charged.

  • Treat Neuro Arts WA staff and other clients with courtesy and respect and ensure the same from their representatives, family members, and support persons.

  • Notify Neuro Arts WA if a new Nominee or Plan Management Provider is appointed.

  • Inform Neuro Arts WA of any personal changes impacting the services, such as changes in residence or medication.

  • Notify Neuro Arts WA immediately if their NDIS Plan is suspended, replaced, or if they cease being an NDIS participant.

  • Inform Neuro Arts WA if they wish to cease engagement with the service.

4.2 Responsibilities of Neuro Arts WA

Neuro Arts WA agrees to:

  • Comply with NDIS Codes of Conduct in providing services.

  • Regularly review the services with the Participant and, as applicable, their representative.

  • Deliver services that meet the Participant’s needs at their preferred times.

  • Communicate openly and honestly in a timely manner.

  • Treat the Participant with courtesy and respect.

  • Consult the Participant on decisions about how services are provided.

  • Provide information about managing complaints or disagreements and details of the cancellation policy.

  • Listen to feedback and resolve problems quickly.

  • Provide 14 days notice if Neuro Arts WA needs to end the service.

  • Protect the Participant’s privacy and confidential information per applicable laws, including the Privacy Act 1988 (Cth).

  • Provide services in a manner consistent with applicable laws, including the NDIS Act, NDIS Rules, and the Competition and Consumer Act 2010 (Cth).

  • Issue regular invoices and statements for services delivered.

5. Changes to Services

Any changes to the services, their delivery (including fees), or this Service Agreement will be discussed and reviewed by both parties.

6. Ending the Service

To end the service, either party must provide at least 14 days notice or a longer period to enable the Participant, their nominee, or NDIA to nominate an alternative provider. If either party seriously breaches the terms, the notice requirement is waived.

7. Complaints and Disputes

Participants have the right to provide feedback, make suggestions, have grievances heard and considered, and commend good performance. Feedback or complaints can be made
